Insurance Considerations
While we fully understand the financial challenge this presents to some patients, Cohesive SOULutions does not bill insurance companies directly for several reasons:
For insurance to cover treatment, you must be officially diagnosed with a mental health disorder. We don’t believe our clients need to be pathologized in order to derive life-long benefit from therapy. A mental health diagnosis in your records may follow you through life and impact you in ways you might not anticipate.
Confidentiality of sessions at Cohesive SOULutions is guaranteed by law except in a few circumstances. One circumstance allowed by the HIPAA Privacy Rule gives health plans and self-insured employers permission to obtain information without your consent for billing and health care related matters. This includes therapy notes! We don’t believe your deepest personal relationships, secrets, and struggles should be an open book for an insurance representative behind a desk, or your employer. Do you?
Cohesive SOULutions believes that the course of therapy should be determined together by the client and their highly experienced and licensed therapist, but when clinics bill health insurance companies directly, the clinicians are required to become participating providers. The therapists must sign a contract that allows the insurance company to determine which services they will and will not provide.
A participating provider must agree to accept the fees the insurance company establishes, regardless of whether the fees are reasonable or applicable. Therefore, therapists who are participating providers must accept discounted fees for their services, and they cannot bill the patient for the difference between their actual fee and what the insurance company will pay. Therefore, the clinic must write off as much as 50% or more of the therapist’s fee for service. At the same time, the participating provider’s office overhead costs have increased dramatically due to the staff, time, and incidentals necessary to process and track claims. In today’s healthcare environment, the actual cost for therapists to provide services continues to rise, while the percentage of reasonable fees that insurance payments cover is declining. At the same time, the profits of health insurance companies and the salaries of their top executives continue to rise to record levels.
Many of our clients have high deductible insurance plans that only cover mental health services after the client has spent hundreds or thousands of dollars out-of-pocket. Cohesive SOULutions uses a Solution Focused Brief Therapy model, so for many clients, that means seeing a participating provider exposes them to all of the risks above, and ultimately, they pay the same amount out of pocket as they would just paying for therapy without involving insurance.
